JOB DETAILS:
*Masters degree is required
*Licensure required
*CPR certificate (health provider)
*Two (2) years relevant experience is required
*Offering a per diem/part-time position

JOB SUMMARY:
Provide patient care consisting of evaluation, planning, treatment and documentation. Promotes and develops physical therapy services in homecare/hospice.

MINIMUM QUALIFICATIONS:
Education: Master’s Degree in Speech/Language Pathology.Licensure/Certification/Registration: Current MN license.

PREFERRED QUALIFICATIONS:
Experience: Previous Home Care experience. Licensure/Certification/Registration: N/A

SPECIAL KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of: speech therapy principles; Experienced with evaluation and treatment of patients with neurological disorders, conducting Swallow assessments and current Dysphagia management. Extensive knowledge in the field of Physical Rehab, including patient assessment, treatment planning and documentation guidelines; knowledge of safety practices and procedures. interpersonal dynamics; community resources; Make decisions and solve problems; work in a mature constructive manner; professional judgment and to accept responsibility; follow through with directions; organize and prioritize responsibilities; present information to others; be self-motivated and independent; time manage.

PHYSICAL DEMANDS/LIFTING REQUIREMENTS:
Prolonged, extensive, or considerable standing/walking; intermittent exertion when performing treatments. Ability to lift up to fifty (50) pounds; sit and drive (providing own vehicle).

AGE SPECIFIC COMPETENCIES:
Skilled at assessment and knowledgeable of growth and development. Provides appropriate care respective to the ages of the patients served per clinical environment.

WORKING CONDITIONS:
Contact with patients under wide variety of circumstances; may be exposed to/occasionally exposed to patient elements; exposed to unpleasant elements (accidents, injuries and illness); subject to varying and unpredictable situations; handles emergency or crisis situations.
